NSW:Drug warnings after elderly patients died

SYDNEY, Aug 10 AAP - The deaths of two elderly patients who were treated by the same
doctor, now deregistered, has prompted a warning about the use of neuro-muscular blocking
agents.

NSW State Coroner Mary Jerram made a string of recommendations on Wednesday after handing
down her findings at the inquest into the deaths at a small NSW rural hospital.

Their treating doctor, Dr Balaji Rao, already has been found guilty of unsatisfactory
professional conduct and professional misconduct and in 2010 was deregistered for at least
a year.

Ms Jerram found that Ralph Leslie Grenfell, 69, died on October 5, 2005 at Tocumwal
Hospital, close to the NSW/Victorian border.

He died as the immediate result of the administration of Atracurium and Pancuronium
delivered intravenously without ventilatory support by Dr Rao.

The coroner found that Arthur Robert Cooling, 88, died at the hospital as the immediate
result of the administration of Atracurium delivered intravenously without ventilatory
support by Dr Rao.

"Atracurium and Pancuronium are forms of non-depolarising neuro-muscular blocking agents,"

she said, adding they cause muscle paralysis.

"A patient who receives those drugs in the quantities in which they were ordered -
in the absence of ventilatory support - will die within minutes."

Ms Jerram twice suspended the inquest so the Director of Public Prosecutions (DPP)
could consider laying charges over the deaths.

But the DPP declined to lay charges.

Ms Jerram said the drugs were "dangerous" and "have no role to play whatsoever in palliative
care".

"Any use of these drugs without mechanical ventilation and appropriate sedation risks
a patient enduring an agonising death of suffocation," she said.

"To the outsider the patient would seem still and at peace but in reality the patient
would be conscious, fully aware they cannot breathe but unable to move."

She recommended a NSW Health Safety Notice be issued warning doctors and nurses of
the potential negative effects of neuro-muscular blocking agents, and that they should
not be used without appropriate ventilator support.

Other recommendations included that Visiting Medical Officers be strongly encouraged
to avail themselves of specialist resources for palliative care and pain relief.

Meanwhile, Mr Macfarlane said current official interest rates were neutral in their
impact on the overall economy.

He said the current official rate of 5.50 per cent was also having a neutral impact on inflation.

"At the current level of 5.5 per cent the cash rate is in line with its average over
the low inflation period since 1993," he said.

"There is, therefore, a sense that the current level is relatively neutral in terms
of its impact on economic activity and inflation."

Mr Macfarlane said a complicating issue was that interest rates on offer to borrowers
were actually below average.

"One complication in assessing the level of interest rates however is that competition
in the financial sector has seen margins between the cash rate and institutions' lending
rates narrow over recent years," he said.

"The interest rate faced by borrowers are still a little bit below average."

sector desperately needs a shake-up .. estimating recent restrictions have cost consumers

Queensland Water Minister STEPHEN ROBERTSON says his government has done the right
thing by investing in desalination infrastructure.

He's told the ABC .. if it hadn't been for the desalination plant .. southeast Queenslanders
would've been boiling water for weeks .. because of damage to treatment plants during
the floods.

Qld: Man saves dog from fast flowing water, winched to safety

A Queensland man who jumped into fast flowing water below a dam wall to save his dog
has been rescued by helicopter .. after spending the night on a rocky outcrop.

Emergency services says the man's wife raised the alarm at about 10.30 (AEST) last
night .. after he became stranded on rocks at Burdekin Falls Dam at Ravenswood .. south
of Townsville.

The 22-year-old man jumped into the fast flowing water to save his dog .. and the pair
became stranded on an outcrop about 200 metres below the dam wall.

Conditions were too dangerous to reach them last night .. so emergency teams managed
to get the man a blanket and food.

A helicopter has winched the man and his dog to safety .. uninjured .. around 6.30 this morning.

NSW: Judge orders serial sex offender's release from jail

A New South Wales Supreme Court judge has ordered the release of serial sex offender
JOHN ALAN WILDE.

The 46-year-old has been in near-continuous custody since 1981 for a string of rapes
.. including the assault of a young mother in front of her two children.

He was last jailed in 1993 for 16 years after forcing his way into a Sydney woman's
home and ordering her to perform sex at knife-point .. just 13 days after he was released
on parole.

It was the third time WILDE had committed such an offence .. having already served
jail terms for home invasions and rapes committed in 1981 and 1983.





At the New South Wales Supreme Court .. Justice MICHAEL KIRBY verbally ordered the
supervised release of WILDE today.

The formal judgment will be handed down at noon tomorrow.

In January the Attorney-General's Department told the New South Wales Supreme Court
it had no confidence in the defendant's safe release into the community.

DILI, Feb 14 AAP - Hundreds of supporters are expected to attend the funeral later
today of slain East Timor rebel leader Alfredo Reinado, remembered by his father today
as a defender of justice.

Dozens of mourners have already gathered at the compound of Victor Alves' Dili home,
many wailing and crying in an outpouring of emotion for his dead son.

Reinado's coffin rests alongside that of another rebel killed during Monday's failed
bid to assassinate East Timor President Jose Ramos-Horta.

Pictures of the two dead men in their military uniforms sat on top of the caskets.

"You are dead," one women cried. "Now you can't talk again. My brother."

Mourners, many wearing torn white ribbons around their wrists, sang hymns and prayed
for Reinado's soul.

Alvez struggled to contain his grief as he spoke to reporters ahead of this afternoons burials.

"I feel so sad, we cannot accept this tragedy," he said.

"It's difficult for us to believe because Alfredo and (President Ramos-) Horta did
a great job. There was never any contradiction between them.

"We believe Horta is someone that will resolve these problems. I still don't believe
this has happened."

Reinado was among 600 mutinous soldiers dismissed by East Timor's government in 2006,
triggering violence in which 37 people were killed and more than 150,000 people forced
from their homes.

Ramos-Horta had taken a moderate approach to the fugitive leader, insisting on talks
to resolve their dispute and waiving an arrest warrant for Reinado on charges related
to 2006 violence.

Alvez said his son's actions would be judged by God, and again urged Reinado's supporters
to refrain from violence.

"So I again announce to all, Alfredo is dead, with what he did wrong and what he did
right ... so just God knows which one was wrong or right."

Alvez said hundreds of supporters and friends were expected to gather for the burials
"because for them, Alfredo is a man who defended justice".

"He fought for them, for what they wanted, so they have come here to show their feelings
as friends."

He said Reinado had answered his country's call to serve his country in army.

"They named him major," Alvez said.

"We agreed when the government took him alive to serve the country, but in the end,
they brought a dead body back to us.

"If he didn't go to serve his country, maybe he could still serve his family."

Security is tight in Dili today, with a Portuguese army tank stationed at the end of
the road leading to Reinado's family home.

Portuguese troops, armed with machine guns, were also patrolling the area alongside police.

The funerals are not expected for some hours yet, with workers still digging the graves.

Elsewhere in Dili, people were going about their normal activities at schools and local markets.

But some fear the situation could deteriorate later in the day, after the funerals
have been held.

Graffiti reading "Viva Alfredo" formerly visible across town has been painted over.

The slain rebel leader was a thorn in the side of East Timor authorities and security
forces, but enjoyed wide support in some sections of the community.

Reinado's wife was not expected to attend the funeral and is believed to still be in
Australia, where she lives with her three sons.

Reinado will be buried within the compound of his house, near his father's home.

A Sydney court's heard a man accused of murdering luxury car dealer BOB LJUBIC was
heavily in debt .. and wanted to extort money.

A witness only known as X .. who's admitted to helping in an alleged plot to kidnap
Mr LJUBIC .. has told the NSW Supreme Court 34-year-old JASON CLIVE MCCALL owed 100 thousand
dollars.

MCCALL'S standing trial for the murder of Mr LJUBIC .. whose body was found in waters
off the southern Sydney suburb of Kurnell in March 2005 .. five days after he disappeared.

X says he was kidnapped .. bashed and interrogated by a gang of men .. after stealing
two Porsches from Mr LJUBIC'S car dealership .. in the harbourside suburb of Mosman in
the mid-1990s.

He says he wanted to get even with Mr LJUBIC .. but denies the kidnap plan involved
him being killed.

The hearing's continuing.

WA: Two men charged with having sex with 14yo, taking photos

PERTH, Dec 11 AAP - Two Perth men repeatedly had sex with a 14-year-old girl after
befriending her on the internet, police allege.

Police charged the pair, aged 37 and 34, after executing search warrants today in Hocking
and Connolly, suburbs in Perth's north.

Police allege one of the men met the girl online.

Later, on November 8 and 9, he and the other man met the girl, engaged in a series
of sex acts with her, and took sexual photographs of her.

The 37-year-old Hocking man has been charged with five counts of sexual penetration
of a child, six counts of indecent dealing with a child, three counts of making an indecent
recording of a child, and encouraging a child to engage in sexual behaviour.

The 34-year-old Connolly man was charged with 11 counts of sexual penetration of a
child, seven counts of indecent dealing with a child, and encouraging a child to engage
in sexual behaviour.

Both are expected to appear in Perth Magistrate's Court tomorrow.

NSW: Brown jury retires to consider verdict

SYDNEY, Aug 4 AAP - The jury hearing evidence in the murder trial of Sydney security
guard Karen Brown has retired to consider its verdict.

The jury retired at about 12.30pm (AEST) today, two weeks after Brown's trial began
in the NSW Supreme Court.

Brown, 42, is accused of shooting dead William Aquilina after he beat her with a knuckle-duster
and robbed her outside the Moorebank Hotel, in southwest Sydney, in July 2004.

Brown has pleaded not guilty to the murder of Mr Aquilina and not guilty to an alternative
charge of manslaughter.

Mr Aquilina, 26, was shot dead in a stolen car outside the Moorebank Hotel on the morning
of July 26, 2004.

Seconds earlier, he had beaten Brown with a knuckle-duster and dragged her along the
ground, before stealing a backpack containing about $43,000 in takings from the hotel
and a nearby restaurant.

Queensland's lost its long-cherished claim to be Australia's lowest taxed state or territory.

Treasurer ANNA BLIGH told state parliament Queensland has the lowest tax collection
per person of any mainland state .. which puts it behind Tasmania.

Opposition leader LAWRENCE SPRINGBORG pounced on the admission .. saying Queensland's
always hung its hat on the fact it's the lowest taxed state or territory in the country.

Mr SPRINGBORG says tax increases have come from payroll .. land tax .. stamp duties
.. royalties and gambling bonanzas.

But Ms BLIGH'S defended the government's position .. saying Queensland's abolished
four taxes in the past 18 months.

It's been revealed .. KERRY PACKER's father offered US president RICHARD NIXON unlimited
use of his media empire .. after the Americans resumed bombing of Vietnam at the end of
1972.

Fairfax newspapers say a document unearthed from the NIXON archives shows that Sir
FRANK PACKER sent representatives to express his unqualified support for the president
in January 1973.

A letter from NIXON's communications officer says Sir FRANK was disturbed at new prime
minister GOUGH WHITLAM's comments .. and wanted the president to know they weren't reflective
of the majority of Australians' feelings towards the US.





The letter is among papers unearthed in the presidential libraries by STEPHEN STOCKWELL
.. associate professor in journalism at Queensland's Griffith University.

The collaborative community: improving Marine Corps IT health.

Ever had a problem and thought, "I'm not the first person to have this problem; someone must have the answer?" If you're like me, it happens pretty often, and it got me thinking. It has been written that the average knowledge worker generates 2 to 3 gigabytes of information annually. How much of that information can help in your work? How much of your 2 to 3 GB of data could help others? Collaboration and information sharing are necessary pillars for building a true community of professionals. By communicating through a variety of methods and platforms, we can help one another solve our problems and increase the value we provide to the enterprise.

Since 1990, the ability to collaborate and share information has changed radically. In the 1990s, your ability to collaborate was limited. An organization presented not just an administrative boundary, but also an informational one because you only had a telephone, and basic e-mail in some cases, for communication. You interacted only with people you knew, and that usually meant seniors, peers and subordinates within your organization. You had a computer, and you stored any content you created on your hard drive. Any value you generated (ideas, documents, etc.) was realized at the organizational level. As the Internet increased in popularity and reach, this paradigm started to change.

Information now flows across organizational boundaries. People you don't know are instantly accessible through blogs and discussion boards. Content that you create can be uploaded to SharePoint, posted on YouTube, and shared with others. Most importantly, there is the potential to realize the value you generate across the enterprise. Today's technology enables us to break down the barriers to information sharing and improve our "information health," a term the CIO Executive Board defines as the quality, trustworthiness, timeliness and ease of access to information.

In the Marine Corps information technology community, we want to realize value across the enterprise by improving our information health. How do we get there? I believe there are three environmental factors that influence this process. First, we have an occupationally diverse community. We have the information technology management series (2210), telecommunications personnel (0391), computer engineers (0854) and scientists (1550), computer operators (0332) and technicians (0335), as well as librarians, library technicians and the technical information services series (1412).

The wide array of occupations means a wide array of information and knowledge is generated. Some of this information will be series specific, but what remains to be determined is how much of that knowledge can transcend series boundaries. For example, DON librarians, who are part of the 1412 series, could be breaking new ground in information management. However, because we are not sharing knowledge across series boundaries, their expertise may be benefitting just a small slice of the Marine Corps community. In addition to a wide array of occupations, we have a geographically diverse community. From Okinawa to Europe, civilian IT professionals are individually generating 2 GB of information. This abundance of information was a barrier to improving information health. But today's technology allows a computer operator at Marine Corps Air Station Iwakuni, Japan, to get an answer to a question from an IT specialist in Pensacola, Fla.

The Marine Corps IT community is certainly not the only stakeholder (or potential stakeholder) in our information health or in the methods we will use to improve it. Other stakeholders might include Marine Corps organizations, such as Manpower and Reserve Affairs, Training and Education Command, or other Marine Corps communities of interest. Indeed, the list may include audiences external to the Marine Corps. As we move forward, we will want to carefully consider the effects of these environmental factors and leverage any strengths we identify.

The Marine Corps IT community is targeting a number of platforms to increase information health. First, we are increasing the size of our access list to the community's SharePoint site. By the end of this year, we hope to give virtually all community members access. Further, we need to better leverage the existing capabilities that SharePoint offers. Second, our public-facing website (www.marines.mil/unit/hqmc/c4/itmcoi) will transfer to the Armed Forces Public Information Management System by this summer. The migration will allow us to completely update the format and page structure. Once the migration is complete, we will overhaul the content. Third, we debuted our community Facebook page (search for "Information Technology Management Community of Interest") and our Twitter account (http://twitter.com/USMC_ITMCOI) in January 2011. These platforms give us vehicles to pass information and also to engage in two-way dialogue. Finally, we're going to expand our presence on milSuite, the U.S. Army-sponsored collection of online tools that promotes workforce collaboration and secure information sharing behind the Department of Defense firewall. MilSuite (https://www.milsuite.mil/) provides an array of tools that can facilitate knowledge sharing.

The Marine Corps IT community will reap tremendous benefits from improving our information health. In a fiscally restrictive environment, we need to be looking for ways to get more value from our resources. If we create an environment of information and knowledge sharing, we can save the time required to solve our problems, and time is money. Leveraging the technology at our disposal, we can break down barriers to information sharing and realize value at the enterprise level. In the future, this will be critical to maintaining a vibrant community and being "information healthy."

`Jane' steams ahead, but Navy plays no part: Film about female SEAL wounds reality.(A)

It may rank among the most evasive maneuvers: The U.S. Navy just said no to Demi Moore when she came looking for help with "G.I. Jane," which previews in theaters this weekend.

The film features Miss Moore as a warrior goddess in crew cut and camouflage, determined to be the first female SEAL despite "weeks of physical and emotional hell" and a conspiracy among higher-ups, according to production notes from Disney-owned Hollywood Pictures.

The film, said screenwriter-producer Danielle Alexandra, "was written by a woman, for a woman, about a woman." It "will inspire women inside and out of the military to be fearless, forgiving and committed to the limitless nature of their possibilities."

That's an interesting notion in the year of ousted combat aviatrix Kelly Flinn, questions over sex-integrated training, sexual harassment hot lines and a Pentagon survey that found only 22 percent of male officers believe that women should be treated as men in serving in combat.

The official "G.I. Jane" Internet site offers a history of women in the military, definitions of "combat support" vs. "combat" and an interactive "Sound Off" area.

But Navy blessings for "G.I. Jane"? No can do.

"We pride ourselves on the fact that individual dignity is primary in the U.S. Navy," said Rear Adm. Kendell Pease, the Navy's chief of information. "We felt that the script we reviewed didn't reflect today's military. Our objective, when we give cooperation with a film, is realism."

The reality is simple: There are no women in the SEALs.

"Top Gun," "The Hunt for Red October," "Air Force One" and even comedian Pauly Shore's "In the Army Now" won military cooperation, but "Jane" was left at the dock.

"Largely because of scenes involving harassment, the Pentagon blew off Demi Moore for her new project `G.I. Jane,' " Norma Meyer wrote in a dishy Copley News Service story about Hollywood and the military.

The project became more soap opera than action flick.

The "miffed" Miss Moore called President Clinton in March last year, hoping for White House muscle. The SOS call got as far as an aide in Cabinet affairs and was routed to the Pentagon.

White House spokesman Michael McCurry kept a happy face. The president, he quipped, was upset over the incident, particularly when he heard it involved Miss Moore "in a diving uniform."

"I've now given them my phone number," Mr. McCurry said.

What exactly did she want? Navy "cooperation" meant shooting the film at Navy facilities, with Navy personnel and equipment. It never happened. Miss Moore got a "visit of opportunity" to see SEALs training in San Diego last year and, according to a Navy source, technical advice by phone.

The production team eventually faked a "secret naval base" on a state-owned military reservation in Florida, complete with black-painted barracks, 50 tons of trucked-in sand and some ex-SEAL advisers.

"Remember, this is a movie," Adm. Pease said. "It is entertainment, not fact."

Bemused observers note that "G.I." is most often associated with foot soldiers.

"Navy people," a Navy retiree said, "get called `swabbies.' Hollywood probably couldn't deal with `Swabby Jane.' "

The Navy doesn't want hard feelings, though.

"We were impressed with Demi Moore's quest. Our dealings with her were pleasant," said a Navy officer who asked not to be identified. "But we really scratched our heads over some of their stuff."

The film had more troubles. Hasbro, the maker of G.I. Joe and, come October, G.I. Jane dolls, refused producers permission to use the name.

Writers bandied about alternatives such as "A Matter of Honor," "Pledge of Allegiance" and "Navy Cross," but they didn't have the same cachet.

Money finally talked. The production notes say: "The title `G.I. Jane' and all references to the name `G.I. Jane' are under license from Hasbro Inc."

If anything, audiences will remember the startling sight of Miss Moore in the one-eighth-inch-long regulation haircut of the special forces. She gets the big trim on camera.

"It's about neutralizing her sex to be matched with the men," she said last year before her close shave. "She knows she'll never be accepted if she bunks alone and looks too girlish. She has to shower with them and everything!"

Mubarak fined [pounds sterling]20m for web ban.(Features)

GameStop Announces Agreement to Acquire Kongregate Inc.

GameStop Corp. (NYSE: GME), the world's largest video game and entertainment software retailer, announced it has entered into an agreement to acquire Kongregate Inc., a leading social gaming destination and community for core gamers representing the fast-growing free-to-play gaming market. Credit Collections: Fair Isaac, Online Resources Are at the Door: The Web banking firm is teaming up with Fair Isaac to expand Web-based delinquency collection tools.

The credit clock is ticking along toward full-blown crisis, if it isn't already there. The table is set for technology firms looking to win business from creditors who are hungry for new debt collection and management solutions.

One of the early movers is Online Resources, which is partnering with Fair Isaac to make its Web-based collections tool available to hundreds of collection agencies and debt buyers via Fair Isaac's ScoreNet network.

The partnership will allow ScoreNet users to access an Internet connection product while providing an easier exchange of account data between the two firms. ScoreNet is one of the largest communications platforms in the collection industry and provides managed access between more than 1,300 creditors and third-party business providers. The two firms' connection links all parties in the debt collection chain.

Online Resources' suite includes its Virtual Collection Agent, a 24/7 web product that works to reduce losses from delinquent consumers accounts and CollectPay, an integrated online payment service that increases consumer options while lowering processing costs. "Online Resources provides us with a rules-based decisioning point, where a debtor can set up a payment, or set up frequencies of payment," says Jerry Arehart, client partner of business services for Fair Isaac, which counts the market's 100 largest credit issuers among its client-base. The idea is that centralizing will increase collections plus increase adoption of lower-cost automated channels among debtors.

Online Resources' Virtual Collection Agent has been available for two years, and is used by five of the largest 25 card issuers in the U.S. The firm says that according to the product's past performance record, a client with $50 million in delinquent outstandings showed a loss reduction improvement of 310 basis points, or an annualized savings of about $3.1 million.

Another user, a national collection agency, experienced liquidation rates of three times greater than the portfolio average compared to traditional collection methods. And that user's Website visitors also paid at a rate four times greater than the average. Additionally, debtors that use the Web to "self-cure" their delinquent status had a 57 percent higher balance than users of other collection methods. Almost half of the Web site users had never been contacted manually and two-thirds were "late stage" accounts. The firm believes the partnership with Fair Isaac will be of particular benefit for subprime lenders, since many of these lenders do not have right-party contacts for their delinquent accounts, and thus rely on self-use channels such as the Internet.

The collecting of overdue and delinquent bills may be the one time that most people prefer the impersonal anonymity of the Internet to a one-on-one personal customer service session.

Beyond its function as a collector of delinquent debts, the systems works like a CRM system. Debtors log-in to a creditor's Website, identify themselves though an account number and are able to access information that's commonly sent through a collection letter or phone call. "You can see what strategies have been deployed for your account, such as an offer to pay over a set number of periods or if offers were made to you at a discount," says Bob Craig, evp and general manager of the electronic commerce unit at Online Resources. "There's a great opportunity for people to cut their debt and settle obligations, as well as negotiate those obligations, in a remote channel that's depersonalized," Craig says. "There's a fair number of folks that prefer that kind of independent interaction."

AMR CEO Vows to Give Unions Complete Confidence During Restructuring; Carty Again Apologizes to Union Leaders, Employees.

AMR Chairman Don Carty today publicly apologized to American Airlines employees, saying that he had made a "big mistake" in his previous talks about executive compensation with union leaders and launching a new round of briefings to help give unions "complete confidence" as the airline's cost restructuring process goes forward.

"You know, the world's largest airline doesn't do things halfway. When we do something, we do it bigger and better than anyone else," Carty said in a statement. "We did what has never before been done . . . we delivered the largest consensual savings in U.S. history. And then I made a mistake and, of course, it was a big one."

Last week, Carty announced that he and his senior management team had given up their planned retention payments to further demonstrate their commitment to "shared sacrifice" as the company works to reduce employee costs by $1.8 billion a year.

Carty emphasized that there was nothing improper about the retention agreements, only in the way they were communicated.

In addition, Carty said the errors were his own, and should not reflect upon the AMR Board of Directors.

"Our Board will retain its historical practice of ensuring that American - - and AMR -- are conservative and responsible in all financial matters, including compensation structures," Carty said.

Today, the company began a new round of discussions with labor leaders to answer their questions about executive compensation agreements and help address any misunderstandings about the company's 10-K filing.

Carty said union leaders "must have complete confidence in the fact that the sacrifices are indeed shared and that there are no more surprises. They deserve the truth and so do our employees."

In the past two years, Carty's total compensation has dropped by more than 80 percent, and he has declined any retention payment or bonus or performance share grant. Of the six major air carriers, Carty is the lowest-paid CEO.

"It is important that all the employees who have been asked to share in the sacrifice understand that despite my mishandling of this particular situation, the board has acted responsibly and I have shared in the sacrifice and my commitment is real," Carty said.

Carty said that AMR remains on the "precipice of bankruptcy," and urged union leaders and employees to stand by the consensual cost-savings agreements ratified last week.

"The precariousness of our financial condition simply can't sustain any action that would delay or prevent the consensual restructuring measures from taking place on schedule," Carty said.

With large payments pending and facing "very real deadlines," Carty said that the company must immediately implement these agreements or be forced to file for Chapter 11 protection.

Bankruptcy would force more aggressive cost cuts, including an additional 10,000 jobs, further pay reductions and a significant threat to employee pensions.

Carty said that employees had worked too hard to stave off bankruptcy to abandon the consensual agreements now, and said that he would work hard to repair any damage he had caused to the spirit of cooperation and collaboration he is trying to build at American.

"We've come this far because everyone has pulled together to make the tough choices and do what is necessary to keep this great company of ours out of bankruptcy," Carty said. He vowed to work to "build a bridge back to the path that allowed us to forge these historic agreements in the first place."
